Bumping this up-- addendum to what I said earlier about the Emergency! pilot from 1972: the ad I posted was for the series premiere of it, and not the pilot picture. Here's an ad for the pilot of it, this one from what was formerly WWJ Channel 4 in Detroit (now WDIV, and still NBC there on Channel 4):

Maybe not the best, but the first episode of Seinfeld was notable in that Julia Louise Dreyfus was not in the cast.
There was a waitress character, who I thought could have been developed more.
